This notice describes how your private healthcare information (PHI) can be used and disclosed and how you can get access to it. It is presented on behalf of all independently practicing healthcare providers at NorthWest Ohio Psychological Services.

It describes your rights for privacy and use of PHI, our responsibilities for use and disclosure of PHI, and your remedies if we do not get it right.

Notice of

Good Faith Estimate

​

If you will pay for health care out-of-pocket because you do not have insurance or do not want to use it, the No Surprises Rule of the Public Health Service Act requires health care providers to give you an estimate of expected charges at the onset of services, or if you ask for an estimate. This is called a Good Faith Estimate.

​

It describes the timelines, process, and requirements for the GFE, and your remedies if we do not get it right.
